Lost documents: what to do

If Aadhaar, PAN, a driving licence, marksheets or a passbook are lost, first lodge a loss report with the police (its receipt is needed for duplicates), then ask the issuing authority for a duplicate. Many documents are available at once from DigiLocker.

What to do

  1. If a bank card, cheque book or SIM is lost, call the bank or operator first and get it blocked.
  2. Lodge a loss report at the nearest police station or on the Uttarakhand Police citizen portal, and keep the receipt.
  3. Aadhaar: download e-Aadhaar or order a PVC card from the UIDAI site or app. Driving licence and RC: apply for a duplicate on Parivahan Sewa.
  4. Marksheets and certificates: check the issued copy in DigiLocker; otherwise apply to the board or university for a duplicate (a police report and an affidavit may be asked for).
  5. For a lost registered deed, a certified copy is available from the Sub-Registrar office; a newspaper notice and an affidavit may be needed.

  • Any photocopy or number of the lost document
  • Another identity document and passport-size photos
